The documents required for your appraisal need to be gathered into a portfolio which includes various declarations about health, probity etc. All of the portfolios on this page comply with these requirements. If you choose to use an alternative vehicle it is your responsibility to ensure that it at least replicates the information on the Medical Appraisal Template and that your appraiser can use it and access it. (The Medical Appraisal Template can not be used as a stand alone appraisal form.)
Feedback Surveys Most of the portfolio tools offer the colleague and patient surveys which are required, usually for an extra fee.

The supporting information used for appraisal and revalidation must be anonymised by doctors to ensure that all personal identifiers, including names, dates of birth, addresses, hospitals and NHS numbers, are removed and that patients, carers, relatives and staff are not directly identifiable."

If you are unable to do this for specific items of information (e.g. compliments & complaints) you should log the existence of the information in the portfolio but provide the information itself separately to your appraiser.
