Toolkits and surveys
The documents required for your appraisal need to be gathered into a portfolio which includes various declarations about health, probity etc. All of the portfolios on this page comply with these requirements. If you choose to use an alternative vehicle it is your responsibility to ensure that it at least replicates the information on the MAG form and that your appraiser can use it and access it. Appraisal documents will only be accepted in electronic format (NOT on paper). Handwritten documents are NOT acceptable. You must remove all patient and personally identifiable data. If you are unable to do this for specific items of information, you should log the existence of the information in the form but provide the information itself separately to your appraiser. |
Input-light template 2020/2021
Most of the evidence for the new-style appraisals will be verbal during the appraisal discussion, and will be captured by the appraiser in the appraisal summary, written by the appraiser.
This template should take about 30 minutes to complete and will capture the main points to be covered during the appraisal discussion. Any written documents you may wish to submit are still welcome, but not required. Just attach them to an email to your appraiser. The requirements for feedback surveys at least once every revalidation cycle remains. |

Clarity
Clarity is an appraisal toolkit delivered through a website. Documents can be uploaded to the site and it is straightforward to use. You need to inform the site who your appraiser is, and let your appraiser know you are using Claity. Costs £60 per annum with 25% reduction if RCGP member = £45 per annum. Colleague and patient feedback tools via email are included in the fee. Paper questionnaires are available for the patient survey only as an alternative – the cost of this service (including postal return and anonymised report collation) is £42. |

Fourteen Fish
Fourteen Fish is an appraisal toolkit delivered through a website. Documents can be uploaded to the site and it is straightforward to use. Integrates with CPD learning tracker on univardis.co.uk. Costs £42 per annum or £37.80 per annum if claiming the NHS England South South West's reduction. Colleague and patient feedback tools £21 each if subscibed to the toolkit or £36 if not. There are also discounts available for practice purchase. Some features are completely free. |

CFEP only do colleague and patient surveys and do not offer an appraisal toolkit. |

The MAG/MAF form
This is a pdf file into which documents can be uploaded. It is comprehensive and simple to use once it is open, but opening and saving it can be a challenge! Once complete it can be emailed to your appraiser. The MAG form limits the total capacity for attachments of supporting documents to 18Mb, Always use the current version.
